Breathing Difficulty Emergency: Warning Signs & When to Seek Help

Shortness of breath can happen for many reasons. Sometimes it may be temporary and related to exertion, anxiety, or a minor respiratory infection. In other situations, sudden or severe breathing difficulty can be a sign of a serious medical emergency.

The challenge is that it is often difficult to determine the cause based on the sensation of breathlessness alone. The timing of the symptom, how severe it is, whether it is getting worse, and the presence of other symptoms can provide important clues.

If breathing difficulty is severe, develops suddenly, or occurs with chest pain, confusion, fainting, blue or grey lips or skin, or an inability to speak normally because of breathlessness, seek emergency medical care immediately.

Why Should Breathing Difficulty Never Be Ignored?

Breathlessness can be associated with relatively common conditions such as asthma, respiratory infections, anxiety, or smoking-related lung disease. However, it can also occur with potentially life-threatening conditions such as a heart attack, pulmonary embolism, severe asthma, serious pneumonia, or a blocked airway.

Because several conditions can produce similar symptoms, attempting to diagnose the cause yourself can be risky. Medical assessment is especially important when symptoms are sudden, severe, unusual, or worsening.

Warning Signs That Require Immediate Medical Attention

Go to the nearest emergency department or contact your local emergency medical service if breathing difficulty is accompanied by any of the following:

  • Severe difficulty breathing, gasping, choking, or being unable to speak in complete sentences
  • Sudden or rapidly worsening breathlessness
  • Tightness, pressure, or significant pain in the chest
  • Chest pain that spreads toward the arm, back, neck, or jaw
  • Blue, grey, or unusually pale lips or skin
  • Fainting, severe dizziness, or sudden confusion
  • A very rapid or abnormal heartbeat together with breathlessness
  • Coughing up blood
  • Sudden breathlessness accompanied by pain or swelling in one leg
  • Severe breathing difficulty after exposure to smoke, fumes, or suspected carbon monoxide
  • Choking or a possible obstruction of the airway
  • Severe breathlessness occurring even while resting

These warning signs can occur with several serious conditions and should not be monitored at home while waiting for them to improve.

What Conditions Can Cause Severe Breathlessness?

Asthma Exacerbation

During an asthma flare-up, the airways can become narrowed and inflamed, making it difficult to move air in and out of the lungs.

COPD Exacerbation

People living with chronic obstructive pulmonary disease (COPD) can experience episodes in which their usual breathing symptoms suddenly become worse.

Pneumonia

Pneumonia is an infection or inflammation of the lungs that can cause cough, fever, chest discomfort, fatigue, and shortness of breath. Some people, particularly older adults and those with existing heart or lung conditions, can become seriously unwell and may require hospital treatment.

Pulmonary Embolism

A pulmonary embolism occurs when a blood clot blocks a blood vessel in the lungs. Sudden shortness of breath, chest pain, coughing up blood, or breathlessness associated with pain or swelling in one leg can be warning signs requiring emergency assessment.

Panic or Anxiety Attacks

Panic attacks can produce intense breathlessness, rapid heartbeat, chest discomfort, dizziness, and a feeling of impending danger.

Airway Obstruction or Choking

If food or another object blocks the airway, the situation can become life-threatening within minutes. Severe choking, gasping, or an inability to speak normally requires immediate emergency assistance.

Breathlessness After Smoke or Gas Exposure

Difficulty breathing following exposure to smoke, fumes, or a suspected gas leak should be taken seriously.

When in Doubt, Get Medical Help

Shortness of breath can range from a temporary symptom to a life-threatening emergency. You do not need to determine the cause yourself before seeking help.

If you are struggling to breathe, cannot speak normally because of breathlessness, develop chest pain, become confused or faint, or notice blue or grey lips or skin, seek emergency medical care immediately.
